Request Quarantine/Isolation Letter

We now have an online form that allows you to request the appropriate letter for your employer. There are two letters available.

1. IN Quarantine/Isolation - If you are quarantined, request this letter to inform your employer that you are quarantined. 

2. OUT of Quarantine/Isolation - If you are ending your quarantine, request this letter to be allowed back to work/school
